Cigna therapy coverage in NYC runs through what your plan calls your behavioral health benefit. The number to use is the behavioral health line on the back of your Cigna card, which is sometimes routed through Evernorth (Cigna’s behavioral health subsidiary). Both Cigna and Evernorth contract with our clinical team as in-network providers.

In-network means three concrete things for you: (1) we bill Cigna directly at a contracted rate that is usually lower than full self-pay; (2) you owe whatever piece of the visit your plan asks you to cover, copay, coinsurance, or remaining deductible; (3) you do not have to chase reimbursement, because there is nothing for you to file.

What changes session to session is whether you have met your deductible yet, and what your plan’s copay or coinsurance is for in-network outpatient therapy. Copay, coinsurance, deductible: the three numbers that decide what you pay.

Most of the confusion about Cigna therapy cost comes from people using these three words interchangeably. They are not interchangeable, and which one applies to your plan determines what you pay each week.

Copay

A flat dollar amount you owe per session once any deductible is met. The most common Cigna situation we see at our Midtown practice is a PPO plan with a behavioral health copay, you pay the same amount every week, regardless of what the session “really” costs. Copay plans are the easiest to budget. Once you know the number, weekly therapy has a predictable line item.

Coinsurance

A percentage of the contracted rate (often 10%, 20%, or 30%) that you owe per session once any deductible is met. The dollar amount stays roughly constant from session to session because the contracted rate is fixed, but it is calculated as a share, not a flat fee. Coinsurance plans cost slightly more than copay plans in absolute terms but use the same in-network contracted rate, so the per-session number is still much lower than full self-pay.

Deductible

The amount you have to pay out-of-pocket each plan year before your Cigna benefits start covering sessions. Until the deductible is met, you owe the full contracted in-network rate per session. After it is met, you drop to your copay or coinsurance. Many of our Cigna clients reach deductible by mid-year on weekly therapy, and the per-session cost drops noticeably from that point through year-end. The contracted in-network rate, even while you are paying it in full, is typically lower than the rate you would pay walking into a self-pay practice.

Out-of-pocket maximum

The annual ceiling on what you pay for covered care. Once you hit it, Cigna pays 100% of in-network covered services for the rest of the plan year. People in weekly long-term therapy with ongoing other medical care sometimes hit this number, especially on high-deductible plans.

Four Cigna situations we see most often at our NYC practice.

Every plan is different, but four patterns cover the majority of Cigna members we see at our Midtown Manhattan office and across our online NY State practice. Reading the one that looks closest to yours is the fastest way to know what to expect before the match call.

1. Cigna PPO with a behavioral health copay

You have a Cigna PPO. Your behavioral health benefit pays in-network. You owe a flat copay each session, and the deductible is either low or already met. The most common Cigna situation at our practice.

What to expect: Predictable weekly cost. No surprise bills. No claim paperwork.

2. Cigna high-deductible HSA plan

Your Cigna plan is paired with a Health Savings Account. Sessions before the deductible is met are billed at the contracted in-network rate (lower than self-pay). After the deductible is met, you drop to coinsurance or copay.

What to expect: Cost is highest early in the plan year, then drops. Many clients reach deductible by mid-year.

3. Cigna with Evernorth carved out

Your Cigna plan routes behavioral health through Evernorth (Cigna’s behavioral health subsidiary). The medical card may be Cigna-branded and the behavioral health number on the back may be Evernorth-branded. Our team is in-network with both.

What to expect: Same in-network treatment. We confirm Evernorth as the path on the match call so claims route correctly.

4. Out-of-state Cigna plan, living in NY

You live in NY but your Cigna plan is administered by an employer in another state. Telehealth sessions through our online statewide offering are usually still covered under the behavioral health benefit, because the in-network relationship travels with the carrier.

What to expect: Most out-of-state Cigna members can do online therapy with us in-network. Plan specifics confirmed on the match call.

Cigna vs Evernorth: same in-network relationship, different label.

Cigna and Evernorth are routinely confused. Here is the short version: Cigna is the medical insurance carrier; Evernorth is Cigna’s behavioral health subsidiary. For most Cigna plans, the behavioral health benefit is administered by Evernorth, meaning when you call the number on the back of your card for therapy coverage, you are talking to Evernorth.

Our clinical team is in-network with both. You do not have to pick one. Whether your plan routes claims through Cigna or Evernorth, your therapy with us is billed in-network and you owe whatever your plan asks you to cover.

In-network Cigna vs out-of-network: what changes when you book somewhere else.

You can use Cigna for therapy at practices that are not in-network with Cigna. The mechanics are different. Here is how the same weekly session compares under each path.

In-network with Cigna (Therapy24x7)

  • Billing: we bill Cigna directly at the contracted in-network rate.
